  1. M: Male. BMI: body mass index. CVD: cardiovascular disease (stroke or heart disease). QoL: quality of life. n: subjects with acceptable pre-bronchodilator spirometry and complete information on characteristics, comorbidity, symptoms and QoL. Dyspnoea measured according to mMRC Dyspnoea scale: 0–1 = minimal/no breathlessness, ≥ 2 = significant breathlessness. Presence of chronic cough, chronic phlegm, and wheeze was determined by positive responses to the following questions: 1) “do you cough on most days for as much as 3 months each year?”; 2) “do you bring up phlegm on most days for as much 3 months each year?”; and 3) “have you had wheezing or whistling in the chest at any time in the last 12 months?”. Physical and mental QoL measured using the SF-12 questionnaire. Sites with missing data for SF-12 did not use this tool to assess QoL
