(A) Thickened alveolar walls showed edema, fibrosis, and inflammatory cell infiltration. Dense fibrosis was inconspicuous or absent. (A and B) Intra-alveolar organizing fibrotic areas were seen as pale eosinophilic fibrosis with H&E staining and as Alcian blue positive area with AB-PAS staining as early stage of fibrosis (asterisks). (C) Positive epimorphin immunostaining appeared in areas of early fibrosis (inset in C shows a close-up view; asterisks indicates early intra-alveolar fibrotic area) covered with regenerated alveolar epithelial cells (arrowhead in E), in addition to the alveolar walls. A few α-smooth muscle actin-positive cells were noted in the fibrotic areas (arrowhead in F). Scale bars = 60 μm.
